Test information

Fructose is a simple sugar (monosaccharide) naturally found in many fruits, vegetables, and honey. It is also commonly used as a sweetener in processed foods and beverages, often in the form of high-fructose corn syrup—a mixture of fructose and glucose. In recent years, fructose consumption has significantly increased, largely due to its widespread use in sweetened products.

Fructose that is not absorbed in the small intestine passes into the large bowel, where it is fermented by colonic bacteria. This process can lead to gastrointestinal symptoms such as bloating, gas, and abdominal pain—particularly in individuals with fructose malabsorption. These symptoms are often recurrent and may mimic or contribute to those experienced in irritable bowel syndrome (IBS). Genetic mutations may also increase a person’s susceptibility to fructose malabsorption.

Breath testing is the preferred method for diagnosing fructose intolerance. A standard dose of 25 g of fructose is used as the diagnostic threshold; a positive result at this level indicates reduced fructose absorption capacity. This amount of fructose can easily be consumed through a typical diet, potentially triggering frequent and ongoing symptoms.

Fructose malabsorption should be considered in individuals with unexplained gastrointestinal issues—especially following the consumption of fruit, fruit juices, or foods containing high-fructose corn syrup. It may also play a significant role in the development or worsening of symptoms in people with IBS..
